Kit Processing and Shipping Instructions for Physicians

1. Label the prepared sample with the patient’s first name, last name and date of birth. Then place the sample and requisition form inside the specimen bag. Finally, place the specimen bag back into the collection kit provided.
2. Then place the transport box inside the UPS return shipping bag which has a return label attached. Seal the bag tightly.
3. For Physicians Office’s you choose from the three methods below to ship the kits back to Access.
   • Place your pick up on our Online Portal. Click the “Schedule a Pickup” Tab
   • Drop off samples at a near by UPS store or drop box location. https://www.ups.com/dropoff/?loc=en_US
   • Text us at 561-513-4334 to place your pick up through our Texting Service.
